第三幕：在收费站。黎明，白色的雪覆盖大地。咪咪带着黑色面纱独自一人来到城外的酒馆门前，鲁道夫、马尔切洛与穆塞塔暂时住在这里。咪咪唤人找来马尔切洛，她问起鲁道夫，说他因为嫉妒而怀疑她，对她冷淡。这时鲁道夫也出来了，咪咪赶紧躲在树丛后，他向马尔切洛抱怨咪咪跟别的男人眉来眼去，并说咪咪的病很重，而他没有钱为她治病，所以希望分手，好让咪咪找一位有能力的情人。听到自己得了绝症，不久于世，咪咪痛哭起来，鲁道夫找到她将她拥抱在怀中，咪咪与他告别（咏叹调：我要回到自己的小窝）。酒店里传来穆塞塔与人调情的声音，马尔切洛急忙跑进去，两个人吵起来，恶言相向，最后不欢而散。鲁道夫与咪咪却依依不舍，他们回忆起往昔美好的时光。
